Yan Lin, MS, RDN, LDN, CDCES - I am a registered dietitian and certified diabetes educator based in Philadelphia PA to serve both PA & NJ residents. With extensive clinical experience in hospital settings, senior retirement centers, and outpatient settings, I've had the privilege of witnessing firsthand the transformative power of nutrition on health and well-being. My goal is to provide evidence-based, compassionate nutritional counseling and medical nutrition therapy tailored specifically to your health history and personal goals. I help my clients improve their blood sugar, cholesterol, blood pressure, weight, and other conditions. At Nutritionsrx MNT Counseling LLC, we provide nutrition service through both in person and video conference for your convenience. We are currently accepting major insurance including Aetna, Highmark, IBX, Cigna, Horizon Blue Cross Blue Shield, UHC, Humana, Jefferson Health, PA Medicaid, Medicare , and PPO plans (not accepting Keystone First due to Closed Network). Attention: Medicare only covers Medical Nutrition Therapy for diabetes or chronic kidney disease stage 3 -5 non dialysis conditions. Zero out of pocket cost to you if your insurance covers Medical Nutrition Therapy service. Feel free to reach out to check your insurance eligibility. In addition to 1:1 nutrition counseling for a variety of health conditions, we also provide Diabetes Management Program and Cardiometabolic/Weight Management Program for long term nutrition management of your chronic health conditions.
